Form to join Eastside Arts Coalition as new member

Thank you for your interest in joining EAC. Please fill out the online membership form below.

When you've completed the form, click SUBMIT.
Print your confirmation and send it with your membership dues to
EAC Treasurer, Eastside Arts Coalition, PO Box 3244, Bellevue, WA 98009.

If you are renewing your membership, please mail your check directly to the address above.  Current members will find more information about membership on the Membership Info page.

Once we have approved your membership, we will create a user account for you on this website, so that you can create a public profile page in our Directory. Please provide a name and email address to which the username and password should be sent. If the Website Contact will be the same as the Primary Contact, these fields can be left blank.
